Antisemitic Incident Report Form
 

Please use this form if the incident occurred in Scotland, does not require an urgent police response, and

you have been the victim of an antisemitic incident, or

you have witnessed an antisemitic incident, or

you are reporting an antisemitic incident on behalf of someone else.

In an emergency, always dial 999

If the situation is not urgent, you can contact Police Scotland by dialing 101

This is a joint initiative of SCoJeC and the CST, and has the support of Police Scotland. However, personal information (including your identity) will not be revealed to them without your explicit permission.

When you have filled in the form please save it to your computer, and  e-mail it as an attachment to scojec@scojec.org, or print the form and post it to the address below. If appropriate, please also attach or enclose photographs of the incident or photocopies of any hate mail.
